AVAILABILITY FOR NEW PATIENTS


  • We currently have a 6+ month pending wait list to get an appointment for NEW patients.
  • Our top priority is providing quality care for our patients and therefore we have limited availability at this time.


Steps for New Patients:

  1. Go to our patient portal link and Pre-Register as a new patient.
  2. Log into the Patient Portal and complete all information sections and questionnaires.
  3. Once you have registered for the patient portal, go to the Forms/Reference page to complete/submit our New Patient Paperwork.
  4. Complete the paperwork in full and submit it to our office via email upload, fax, or mail.
  5. Once you have completed all the steps above then you can Book an appointment online.
  6. It may be up to 8+ months before we have an opening. ** If the search option is not showing any availability, just click the send request button and we will schedule you for the first available. Just list your appt preference in the note to the office (day of the week, morning/afternoon). We will do our best to schedule you based on your preferences. **
  7. If you are not able to book online, please send a text message to our office or call to get on the waiting list at 601-450-0953.
  8. If you book an appointment BEFORE you have completed the above steps, then your appointment will be cancelled and have to be rescheduled.

 If you are having problems booking an appointment, you can use the send request button for first available or wait a few hours and try again. Thank you for your patience while we continue to update our booking page.


   Submit your paperwork to us via Email upload link on the Forms/Reference page, via FAX, or via MAIL. 

Please save your original copy in case your documents do not submit successfully.
