HOW TO REGISTER AS A NEW PATIENT WITH OUT OFFICE
WELCOME TO OUR PRACTICE
We are honored that you have chosen us as your health care provider. Our goal is to deliver high‑quality care in a timely and respectful manner.
- Please plan to arrive 15 minutes before your scheduled appointment to allow time for check‑in and to update any necessary information. Your new patient appointment is scheduled for one hour.
- If you need to cancel your appointment, we kindly ask that you provide at least 48 hours’ notice. Late cancellations prevent us from offering that appointment time to other patients, and we appreciate your courtesy.
- If possible, please arrive fasting for your visit. If you are unable to do so, we can either schedule a separate morning appointment for lab work or provide orders for you to have your fasting labs completed at another facility.
- Please bring any prior lab results or medical records you have available so we may add them to your chart.
- Additionally, bring a list of all current medications and supplements, including prescription and over‑the‑counter items. If you are unable to bring the medications themselves, please provide a detailed list that includes:
- Medication or supplement name
- Dosage
- Reason for taking it
- How often and how much you take
- Be sure to bring your photo identification and insurance information to your appointment.
- If labs are drawn during your visit, a follow‑up appointment will be scheduled in 2–3 weeks to review your results depending on the turn around time for the labwork.
- Please use your patient portal to send messages, request refills, review your lab results, access visit summaries, and pay your statements.

Please register for our patient portal using the Patient Portal link. Select “NEW TO PRACTICE / PRE-REGISTER.”
If you experience any issues entering your insurance information, you may skip that step and continue to the next page. We can update your insurance information at a later time. After completing the form, be sure to click “Submit” at the bottom of the page so your information is sent to us.
Please note that your patient portal will not be activated until all new patient paperwork has been completed.

You may complete your new patient paperwork by completing a PDF copy. Completed PDF forms may be returned to our office by email, fax, or mail.
