3 STRIKE POLICY
Please be courteous to other patients who need our care. We will no longer provide your medical care after 3 strikes:
1. Appointment not cancelled 24 hours prior
2. Greater than 10 minutes late for appointment
3. Failure to show
We encourage our patients to be pro-active in their health care and seek needed refills on a timely basis. Because prescription refills require authorization by your Provider, we ask that you allow 48 hours to obtain refills. The following information will be needed:
•Patient's name and date of birth •Patient's phone number •Name and dose of medicine •The frequency the medicine is taken •Pharmacy name, location and phone number
You may also ask your pharmacy to fax us a request for your medication to 601-450-4640
New Patient Form
